The Elevated Membership includes comprehensive initial and preventive visits, routine and sick visits, chronic-condition follow-ups, appropriate telehealth visits, priority scheduling, secure messaging, and member-only wellness benefits.
Laboratory testing, imaging, medications, specialist services, IV hydration, wellness injections, hormone treatment, weight-management services, and peptide therapy are not included unless specifically stated.
MEDICAL WEIGHT MANAGEMENT

Initial Weight-Management Evaluation Package — $450
Includes the initial consultation, baseline laboratory testing, and one follow-up appointment to review results and discuss the treatment plan. Weight-loss medication is not included in the initial package and is priced separately.
