  • The Elevated Membership is our all-inclusive, subscription-based model designed for patients who want direct access, longer visits, priority scheduling, wellness discounts, and exclusive benefits. Instead of dealing with insurance claims, you pay a flat monthly fee for comprehensive care. If you don’t schedule a visit during the month, you may apply the fee toward a wellness service!

  • The Elite Care Path is a traditional option that allows patients to use their insurance for visits while still enjoying longer appointments, personalized care, and a welcoming environment. It’s perfect for those who prefer to use their health benefits without compromising quality. 

  • No membership, no insurance, no problem. With the Empowered Care Path, you’ll pay transparent cash-pay rates for the services you need when you need them. This includes lab testing, physicals, medication management, sick visits, and much more.

  • You can absolutely join! Many patients choose Elevated Membership for more personalized, direct access, even if they have insurance. You can still use your insurance for things like specialist visits, hospitalizations, prescriptions, and lab work.

    • Primary Care (Preventive visits, chronic disease management, urgent concerns)

    • Weight Loss (Nutrition and Medication management to help with weight loss goals)

  • You can schedule online through our website, or call/text us directly. Members receive priority scheduling and direct provider access.

  • We offer easy online self-scheduling for in-person appointments, with occasional same day appointments, pending availability. Simply check our booking link to find a time that works best for you! 

  • Yes! Our memberships are month-to-month with no long-term contracts. We kindly ask for a 30-day notice for cancellations.

  • We do not handle medical emergencies. If you’re experiencing a life-threatening emergency, please call 911 or visit the nearest ER.

  • Yes! We provide comprehensive medication management, including GLP-1 medications such as Semaglutide and Tirzepatide, based on a full medical evaluation and your health goals.
    In addition to GLP-1 therapy, we offer a wide range of other weight loss strategies and services to ensure we can customize a plan that’s best for you.

  • In many cases, HSA/FSA funds can be used for direct primary care memberships. We recommend checking with your plan provider.

  • We cover certain labs with your membership cost. We also coordinate discounted cash-pay options for labs/imaging or bill your insurance. We are partnered with several specialists for referrals and we will guide you through the process.

  • Yes! In fact, We love children! We offer care for adults, children, and families—making healthcare simple and stress-free.

  • Absolutely! Elevated offers secure, HIPAA-compliant messaging for easy communication with your provider. Plus, our patient portal gives you access to your lab results, visit summaries, prescriptions, and other important information, all at your fingertips.
