It is also ideal if you manage an ongoing condition like blood pressure, diabetes, or thyroid and need regular check-ins. For less than the cost of one urgent care visit each month, you get direct access to a provider who knows your history and actually has time for you.
How to join
1. Book your first visit. Choose "New Patient Comprehensive Visit" and note that you want the membership.
2. Enroll. We set up your membership and monthly payment at your visit.
3. Get care. Book, message, and follow up whenever you need us.
WHAT IS NOT INCLUDED
Membership covers primary care and acute care visits with Elevated. Labs, imaging, medications, specialist care, hormone treatment, medical weight loss, IV hydration, injections, and peptide therapy are separate and listed on our Pricing page. Membership does not include 24/7 or emergency care. For an emergency , call 911.
